• your name, address, email, and telephone number;
• a statement of good-faith belief that the use is not authorized by the copyright owner, its agent, or the law;
• a statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act for the owner;
• your physical or electronic signature.

We review valid notices promptly and remove or disable access to infringing material. Repeat infringers' accounts are terminated. Counter-notifications are handled per 17 U.S.C. §512(g).
